Insurance Coverage for Physical Therapy

Does my insurance cover physical therapy?
Most insurance companies reimburse physical therapy and recognize it as a common and necessary treatment for patients.  Insurance companies have reimbursement
plans for both in and out of network preference.  

Why have I chosen out-of-network?
Being out-of-network allows me to provide longer, individualized treatment sessions and maintain a high standard of care.  You and I become the managers of your
treatment plan, rather than a third party intermediary.  This method avoids co-pays and streamlines treatment.  I do my best to keep rates fair and cost effective for people
with no insurance, poor insurance or high deductibles.  

How does it work?  
You will receive an invoice for each paid visit which you can then submit to your insurance company for reimbursement.  The invoice will be provided to you with the
necessary codes and information most insurance companies request. Reimbursements will then be sent directly to you by your insurance company.  All you have to do is
send the already completed form in to your insurance company.  It's that easy.  

How can I find out more about my plan and coverage?
Call your insurance company's customer service number on the back of your card, and ask them to provide you with their reimbursement policy for out-of-network outpatient
physical therapy. Then, find out: your deductible, whether there is a limit to the number of sessions they will cover, and what percentage they will reimburse.  

How often will I be seen for treatment?
Generally, most patients are seen once a week. Your total treatment plan will vary according to your situation.

How long is each visit?
Your first visit is an hour and a half in length.
Follow up visits vary between 30, 60 or 90 minutes depending on what we both decide is best for your situation.

How do I make payments?
Payments can by made on the day of the visit with cash, check, or credit card in person and by credit card online (through your home computer or smart phone). All online
and credit card payments are made through a secure server.

Do you accept Workman's Compensation?
I do not accept Worker's Compensation, Medicare or Medicaid at this time.

Do you work with my physician?
Yes, I work collaboratively with your physician and start open communication with your medical team at the onset of your referral through my evaluation summary of our first
visit.  

What is the Cancellation Policy?  
Missed appointments, changes and cancellations within 24 hours are charged the full session rate.
